Personal Information Protection Act & Media Consent Form 2022-23

  • I consent to having Island Pacific School collect personal information that may include student identification information, birth certificate, legal guardianship, court orders if applicable, parents’ telephone numbers and email addresses, behavioural, academic and health information, most recent report card, emergency contact names and telephone numbers, doctor’s name and telephone number, health insurance number and any similar information needed for registration. I also consent to the collection, use and disclosure of such personal information by and to agents, contractors and service providers of Island Pacific School.

    I further consent to the use and disclosure of information contained in this form and otherwise collected by or on behalf of Island Pacific School

    1. (1) for the purpose of establishing, maintaining, and terminating the student’s or parent’s relationship with Island Pacific School,
    2. (2) for additional purposes identified when or before personal information is collected, and
    3. (3) as otherwise provided in Island Pacific School’s Personal Information Privacy Policy for Parents and Students, a copy of which is available on request.
  • IPS takes may photos throughout the year. The students love to see themselves in the newsletter, on our website and in school slideshows, etc. The photos are typically taken by IPS staff. Occasionally, IPS may use a professional photographer to take photos for marketing related purposes.

    IPS and uses these to create the IPS annual yearbook, expedition slideshows, highlight student work, achievements and school programs in our newsletters, on our website and on school social media platforms. In addition we will sometimes use IPS photos for print materials and advertising. We also post videos, such as Masterworks, on the school's YouTube channel. In addition, we may at times create and use videos for marketing purposes that will be posted on the school's website.

  • The school will prepare a family contact list for the use of Island Pacific School families and staff. This contact list is included with the medical form booklets that travel with student groups on expeditions & excursions. This contact list is also shared with the class reps for communications about volunteering and class parties, etc. This contact list is intended only for use within the IPS school community and contact information is not shared outside of the school community. This contact list will include phone number(s), address and email address(es).

  • This form should be signed by both parents/guardians. However, in some cases, and where legally authorized, one parent/guardian may sign this form on behalf of both parents.
